Frequently Asked Questions 

Prescriptions

We need 48-hour notice for refilling prescriptions, Monday through Friday. In an effort to expedite your request for a prescription refill, please have your pharmacy phone number handy when you call. It is always best to call before you are completely out of medication.

Canceling appointments

If you are unable to keep your scheduled appointment, please notify us at least 24 hours in advance so we can accommodate our other patients. You may also reschedule your appointment at that time. There is now a $50 fee for not showing for an appointment if we are not notified.

Medical records

Each patient has a complete record of all medical care received at our office. Your personal medical record provides a history of treatment, medication, and diagnostic information that enables your healthcare team to make comprehensive medical evaluations. We consider your records to be confidential. Therefore, information will not be released without your written consent, unless required so by law. Copies of your medical record will be released to you or transferred to another physician upon written consent. There will be a $25 - $50 copying fee for this service if you are picking them up. We will continue to send/fax your records to your new physician as a courtesy. In addition, the completion of forms by the physician and his or her staff will be $25 - $50 (workman’s comp, disability forms, etc.)
